Anticoagulant Induced Bleeding


- HEPARIN:
- d/c'ing Heparin (t/2=1.5 hrs), will normalize PT/aPTT;
- serious heparin induced bleeding is treated with PROTAMINE sulfate:
- 1mg PROTAMINE sulfate for each 100 units of HEPARIN;
- WARFARIN: - w/ life threatening hemorrhages give:
- Bank Plasma (2 units at a time)/check PT;
- FFP will also work but is more expensive;
- for Prolonged Reversal of Anticoagulant: Vit K 10 mg PO qd x 3
