Ampicillin (Amcil, Omnipen)

Discussion

  • ampicillin is usually given IV as unasyn vs. amoxicillin which is given orally;
  • for treatment of suseptible gram negatives (Shigella, Salmonella, E. coli, H. influenza, P.mirabilis, N. gonorrhea) and gram positives (streptococci);
  • adult: 500mg - 2gm PO, IM, or IV q6hr;
  • peds:
    • term infants: 150-200 mg/kg/24hr divided q6-8hr IV or IM;
    • > 1 month and children: 150-400 mg/kg/24hr divided q4-6hr IM or IV; 50-100 mg/kg/24hr divided 6hr PO upto 250 mg/dose;
  • meningitis: 400 mg/kg/24hr divided q4-6hr IV;
  • renal dosing:
    • GFR > 90: 500 mg q6hr; GFR 60-90: q8hr; 10-50: q12 hr; <10: q12-24 hr;
    • supplied: capsules 250 mg, 500 mg; suspension 100 mg/ml, 125 mg/5ml, 250 mg/5ml; powder for injection: 125 mg, 250mg, 500mg, 1gm, 2gm, 10gm vials
