- Discussion:
    - defect of the thumb;
    - long neurovascular island has be marked out to replace soft tissue & to increase the blood supply to the relatively avascular thumb;
    - long flap has been marked out on the ulnar border of the middle finger & proximal incision marked on the palm of the hand;
    - flap is drawn beneath the skin bridge, thru a penrose drain, w/o torsion on the pedicle
