- Second Generation Ceph

- Adult: 1-2gm q4-8hr IM/IV;t1/2=.5 hr;
   - or try:1-2gm q6-8hr IV to max dose 12gm/day (2gm q4hr);
   - for surgical prophylaxis:2gm IV/IM 30min prior to surgery then 2gm q6hr for 24hr( 72hr for prosthetic arthroplasty);
- Res to B. Lactamase; good for: B.fragilis, aerobic and anaerobic; N.gonorrhea, and surgery;
- Contra with Cephalo allergy,
- Good diffusion from blood into CSF only with inflammation;
  - Ratio of CSF to Blood Level (%): Normal Meninges: nil;
  - Inflammed Meninges: 50; DosingRegimens for Patients with
- Renal Insufficiency: (Dose for 70kg Adult{gm/dosing interval in hours}): CrCl:>80:1/6-8; CrCl:50-79:1/6-8; CrCl:30-49:1/8-12; CrCl:10-29:1/12-24
   - 85-100 % of dose will be excreted in urine (w/ nl R F(x))
- Note may falsely increae Serum Cr;
- Cefoxitin is unique in providing marked activity against Bacteroides fragilis and penicillin-resistant strains of N. gonorrhoeae
