Synthes IM Femoral Nail Insertion Technique

- Incision and Entry into Meduallary Canal    
- Template Nail Length & Diameter;
      - implant sizes:
      - diameters of 9, 10, 11, 12 mm;
      - lengths from 300 mm to 480 mm, in 20 mm increments;
      - hold radiolucent ruler against lateral aspect of thigh & adjust
            tip of ruler until it is flush w/ tip of greater trochanter;
            - mark the skin at this level, and move C arm to distal femur,
                  replace prox. end of ruler at skin mark & take AP image of
                  distal femur, & read length from distal epiphyseal scar;

- Cannulated Reamer:
  - drill over guide wire w/ 13 mm cannulated drill bit, & drill to minimum depth of 10 cm;
         
  - if using spiral blade, use URFN Broach to enlarge opening to 15 mm,
          to accommodate Locking Sleeve;
- Assemble Nail: verify that proximal locking jig has proper alignment;
- Insert Nail;
        - insert nail to frx site, & use it to assist w/ reduction;
        - pay attention to rotational alignment
        - verify fracture reduction and nail position radiographically;
        - if proximal end of nail is below tip of greater troch, then use proximal end cap
              (can be extended up to 20 mm)
        - release traction;
