Background.--The management of severely displaced, unstable supracondylar fractures of the humerus in children remains controversial. Methods.--During a 20-year period 42 children with severe supracondylar humeral fractures were treated by open reduction through a medial incision and internal fixation by a single transarticular pin. Results.--Thirty-four children were evaluated after an average follow-up period of 6.8 years (range, 9 months to 20 years). In 79% of patients excellent or good results were achieved with loss of less than 11 degrees of arc and with less than 11 degrees of change in carrying angle. All but 2 of 14 patients with nerve injuries recovered fully.
