Pedobarographic studies have shown that patients with hallux valgus have an abnormal pattern of forefoot loading. In these patients, Keller's arthroplasty defunctions the great toe and causes even more imbalance of load distribution. Restoration of mechanical function would probably be better achieved by techniques other than excision arthroplasty. In adolescents and young adults, distal osteotomy of the first metatarsal is a reliable treatment for hallux valgus. However, the situation in the older patient is less well defined. A prospective study was conducted to clarify the role of metatarsal osteotomy in the treatment of adults with symptomatic hallux valgus.
