- treatment of infections: 500mg-1gm IV/IM q4-6hr;
- may use doses up to 12 gm per day for serious life threatening infections;
- pre operative prophylaxis to prevent postoperative infections with
potentially contaminated surgery:
- 1-2gm IM/IV 30min-1hr before start surgery, 1-2gm during surgery,
1-2gm IM/IV q6hr for 24hr postoperatively;
- decrease dosage with renal failure;
